We’ve facilitated several meet-ups for the Society for Participatory Medicine. Join us in December to discuss two projects centered on the Social Determinants of Health and how they impact all aspects of participatory medicine.

Hello Chicago S4PM! We hope that you are all safe & well during these unprecedented times, and stay well through the holiday season.

Our next session will be focused on SDoH issues that need to be addressed. Our two presenters are S4PM members Mark VanderKlipp and Paulo Machado.

• Mark will review the Six Conditions of Systems Change and how they were met in a project at The Intersection of Health and Homelessness.

• Paulo Machado will review how to address Loneliness & Lack of Social Connection in the work he's been doing.

• Our goal is to discuss how to scale SDoH solutions and the hurdles that need to be overcome, both by healthcare providers and patients.

As a primer, we suggest reading Dr Don Berwick’s "Moral Determinants of Health" to help us frame the discussion.
